Dear patients,
The practice has decided to change the way patients request a routine GP appointment; this will be in line with NHS England’s modern general practice model seen below. The new appointment system will start on the 15th April 2024.
You can contact your GP via the website, telephone or by visiting the practice. Your GP Practice will then offer you: a face to face appointment, a phone consultation, a video consultation, a text message, a home visit, or a referral to another service.
We kindly ask patients (from the 15th April 2024) who can use our eConsult service to submit an eConsult with their medical concerns via our eConsult service. Those who cannot use the eConsult service please call or pop into the practice and the care navigators will complete an eConsult Lite on your behalf. eConsult Lite comprises 4 questions the care navigator will ask you.

- What have you tried already?
- Is there any particular help you would like?
Once we have received your eConsult/eConsult Lite our clinically lead triaging team will triage you to the most appropriate healthcare professional or service. To help triage you correctly please put as much information in your eConsult as possible and please answer care navigator questions.
Your eConsult/eConsult Lite will then be responded to by the triaging team (by the method you requested) with an appointment, further advice, or a self-referral link (not an exhaustive list).
Please be aware our urgent care team will not change and ask patients to still call from 8 am if they feel they need to be seen urgently or call 111 outside of our opening hours.
