Registering at Stubbington Medical Practice
If you live in our practice boundary (please see link below) and wish to register as a patient at our surgery, then we need certain information from you and each member of your family to allow us to trace your previous medical records.

Find your NHS number
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality doesn’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
Please allow up to 5 working days for us to process your registration, once processed we will send you a text message (to the mobile number provided) to confirm your registration and to ask for:
- photographic proof of your identity: e.g. driving licence or passport.
- If you do not have a mobile phone, you will be asked to bring the above ID to the practice for verification.
All patients over 16 can register on the NHS app. You can access your medical records, book appointments, order prescriptions and more by downloading the NHS App.
Every patient at the practice is allocated a named, accountable GP. This ensures you have a ‘usual’ GP as a point of contact and to assist with continuity of care. You should always try and see your usual GP, especially for ongoing problems, however we understand this is not always possible which is why there are no restrictions on seeing other doctors.
It is possible to change your named GP at any time, please contact reception who will be happy to help.
Practice Boundary
We have now created an outer boundary around the existing practice boundary to enable patients who move house locally to remain registered with the practice. Our outer boundary is:
Lee-on-the-Solent, Newgate Lane, the River Meon and Meon Shore.
