Mission Statement
Our aims is to provide professional, accessible, high quality, comprehensive healthcare services that inspires confidence in our patients and our community.
The Patients Association
This is a national health care charity that highlights patients’ concerns and needs. It provides advice and news aimed at helping people to get the best out of their health care and tells you where you can get more information and advice. Contact the Patients Association’s helpline on 0845 608 4455 or visit www.patients-association.org.uk.
Age UK (formerly known as Help the Aged and Age Concern) provides information and advice to older people and their families. Contact the Age UK information line on 0800 1696565 or visit www.ageuk.org.uk.
Consent to Investigation or Treatment
Clinicians respect the right of patients with capacity to make or influence decisions about their care. They will discuss and provide information about the available care or treatment and support options so that patients can make informed decisions. Clinicians will ensure that patients have been given enough time and information to make an informed decision, and have given their consent, before starting any investigation or treatment. In the case of minor or routine investigations or treatments, if the Clinician is satisfied that the patient understands what is proposed and why, it is usually enough to have oral or implied consent, ie rolling up a sleeve to have blood pressure monitored. In cases that involve higher risk, it is important to have the patient’s written consent so that everyone involved understands what was explained and agreed.
Clinical Commissioning Group
Our practice is included in the area overseen by the Fareham and Gosport CCG.
Six local GPs on the CCG are in charge of planning and buying health services for patients such as hospital services (planned routine care and emergencies), community services - district nurses and health visitors, and mental health services. The aim is to improve health, wellbeing and healthcare in Fareham and Gosport by encouraging clinicians, managers and communities to work together with patients and the public to transform health experiences and reduce inequalities in an affordable way.
